Contributor Release

I hereby agree to participate in the above-mentioned project and give my consent to the filming, photography and audio recording of my contribution to the project, the nature of which has been explained to me. I give all the necessary consents to the videoing and photography of myself and the recording of my voice and the use of these videos, photographs and recordings for any other purpose, whether by Angel Sharp Media Ltd or by a third person to whom Angel Sharp Media Ltd has granted a right of use, and whether singularly or in conjunction with other photographs and/or videos and/or recordings and in all forms of media throughout the world without further consultation. I understand that Angel Sharp Media Ltd will own all the rights to the footage, video, photography and my contribution to the project. I understand that my contribution will be edited and there is no guarantee that my contribution will appear in the final project. My contribution has, to the best of my knowledge, been truthful and honest. I have not deliberately sought to conceal any relevant facts from the makers of this project. If contributor is under 18: I am the parent/legal guardian of the individual named above, I have read this release and approve of its terms and sign in place of the contributor.
